Temporarily unable to create a new code list

On ALSPAC Archivist, I was temporarily unable to create a new code list:

I replicated the issue on staging using this questionnaire: https://closer-archivist-staging.herokuapp.com/instruments/test/build/code_lists/new

This was happening for me and another user at the same time. Other constructs e.g. questions and response domains were fine.

After a few minutes, and after going out and back in Archivist, we were able to create new code lists. Do you know why this was happening?

This came up again in another questionnaire on ALSPAC Archivist https://closer-archivist-alspac.herokuapp.com/instruments/bib_17_gucc/build/code_lists/70784.

When I entered the code list with one code list value and category it saved first time. Then I was able to add the second value and category. It didn't save when I tried to add both at the same time.
