AI client fetch has no timeout

What

The fetch call to the AI service in ai-client.ts has no signal (AbortController) or timeout configuration. If the AI service hangs, the request will block indefinitely (Node.js undici default timeout is very generous).

Why

While the caller (quizService.generateQuiz) catches errors and falls back to placeholder questions, a hung connection keeps a Node.js event loop reference and socket open. Under AI service degradation, this can exhaust sockets.

Scope

• Add an AbortController with a reasonable timeout (e.g., 30 seconds) to the fetch call
• Ensure the timeout is configurable

Acceptance Criteria

• Fetch calls to the AI service time out after a reasonable duration
• Hung connections are cleaned up
• The timeout value is configurable via environment variable or constant

I've built production TypeScript backends with Fastify that call external AI and LLM services. I've dealt with exactly this pattern — fetch calls that hang indefinitely because upstream services are slow or unresponsive — and learned the hard way that AbortController with a configurable timeout is the only reliable fix. The existing withTimeout utility in src/stellar/resilience.ts is well-structured and I'd reuse it rather than reinventing the wheel.

My approach: wrap the fetch call in generateQuizFromAI with AbortController using a configurable timeout (default 30s). I'd add AI_TIMEOUT_MS to the env schema in config/index.ts so operators can tune it per environment. The timeout error should throw cleanly so the existing fallback in quizService.generateQuiz continues to work as-is. The diff would be small — just the fetch wrapper, the config addition, and a unit test verifying the timeout fires.
