Linea Switch doesn't work

In order to navigate to a specific form, I need to do the following:

1. connect to MetaMask
2. Clicking the Switch Linea button prompts me in MetaMask, but the actual switch does not happen
3. I need to instead switch to Linea chain manually
4. Upon clicking "Leave Feedback" on a specific form

• I get an error: "Application error: a client-side exception has occurred (see the browser console for more information)."
• Image attached of console errors
• 

6. At this point, I still cannot select a specific form without first

• Refreshing the site

Once I have refreshed the site I can then drill down into a specific form and provide feedback.
