SI file: pseudonyms and duplicates

Let's NOT allow multiple entries on any individual. We need to remove VERY CAREFULLY all extra pseudonym entries from the site index:

• Find the entries in the SI that are duplicates @jonhoranic
• check for files that reference these entries
• collapse the pseudonym information into the main person entry, and
• choose the appropriate xml:id to use, and APPLY it to Mitford files.

@jonhoranic You could start a list here of names + @xml:ids in the SI that are duplicates, as in:
Name 1, @xml:id=""; Name 2, @xml:id=""
