examples: sample consumer that queries and displays provider history

Summary

The project's stated purpose is that "a dApp can check a provider's history
before depending on it." There is currently no example of a consumer actually
doing that — only the submission side is demonstrated.

Acceptance Criteria

• Add a small example under examples/ that reads a provider's history
  from a deployed contract and renders it (CLI table or minimal web page,
  whichever is simpler to maintain)
• Display the self-attested caveat prominently in the output — a consumer
  example must not present this data as verified
• Document how to point it at a deployed contract ID
