Screen reader jumps focus when scrolling

I don't know how to reproduce this issue exactly, but when the screen reader scrolls forward and backward, the focus jumps and it even misses entire pages. I am not using Google TalkBack though, but I think this happens on any screen reader. Could you please try fixing this feature?
