dsh-github: GitHub CI, review bot and PR tools for DeepSeek Harness

dsh-github brings GitHub into DeepSeek Harness: a composite action.yml for CI, a PR review bot with idempotent inline comments and a status-check gate, and 12 PR/issues tools — every write gated by human approval, token never logged.

解决什么问题

• 在 GitHub Actions 里用 dsh headless 评审 PR、修复 CI、产出结构化报告（对标 claude-code-action）。
• 轮询评审机器人：按规则（改动范围/测试存在性/敏感文件）评审并发表行内评论，评论幂等；评审结论发布为 status check（成功/需修改），可配置 blocking。
• 12 个工具 + /pr /review /issue 命令族，所有写入走审批门，凭据只引用不落日志。
