Improve order screen and downloaded CSV display of dynamic menu

- Order screen should display ingredients as comma separated, but they are currently joined with no spaces (ex: `"ingredient 1ingredient 2ingredient 3"`)
- CSV should merge dynamic fields with the same titles (maybe also needs to be same input type?)
  - Currently, CSV also has a bug where no data is being displayed under the order fields columns
- Move totals from bottom to right of sheet
