Index does not give enough feedback and ignores items

When running jd index, it does not check your johnny decimal system enough. Some things it should say:

• nested folders: Currently it tries to add the same JD number twice, and then fails, printing an error message. Instead, it should print a warning that there are nested folders, and then continue.
• Improper areas. If you have areas such as 01-10 Label or 11-20 another label, the index works just fine, but then jd cd fails. Index should print a warning message if the areas are improperly named.

The index also marks items with different project numbers but the same other number as duplicates; it saves 1 and does
not save the other one. For example, it thinks that 102.10.02 and 101.10.02 are the same number.
