Root cause identified and fix submitted in #29.

Why this happens:
DeepSeek V4 always returns thinking blocks regardless of whether Codex requested reasoning. Moon Bridge's streamLoop unconditionally emits reasoning_summary_part.added / reasoning_summary_text.delta events for these blocks, but Codex with reasoning effort: none has no active reasoning item to receive them.

Fix approach:
Added a hasReasoningRequested() guard that checks coreReq.Output.Effort, coreReq.Thinking, and the OpenAI extension before treating a block as reasoning. When reasoning wasn't requested, thinking blocks are treated as regular text — preserving the model's thinking content without breaking the protocol.

Additional fix in the same PR:
Also fixed the tool_use/tool_result adjacency issue that causes 502 errors on retry. The convertInput() function was creating consecutive assistant messages that broke the Anthropic protocol invariant of tool_use → tool_result adjacency.

Tested live with DeepSeek V4 + Codex CLI v0.128.0 — all three tool paths (exec, apply_patch, text output) work.
