As a kid I played a lot of Egyptian Rat Screw, it's a good high speed game. A version of the speed aspects could work in this context it would be similar to Otter Stop, but the reverse - instead of reacting to every image until a pause, the player would react only to specific combinations.
The game should show the player a series of playing cards. When two cards in a row are the same value, or a joker appears, the player must click or hit space before the next card is dealt. As the player gets the right answer, the cards will be shown for less time. The game should track a standard 52 card deck, shuffle the card order before the game starts, and again after each pass through the complete deck.
