Admins/moderators should be able to see users ban contest.

- Screen should contain all necessary fields so the mod can analyse the requests.
- Possible ban_contests statuses are: Opened, Closed and Accepted. By default it is
opened.
- In both closed or accepted, column
decision should be present.
