[Bug] Prerequisites fields missing in edit form

Summary

The prerequisite guides fields (existing and todo) are currently visible on the create screen but not on the edit screen.

Steps to reproduce

1. Select a guide
2. Select Edit guide

Expected behavior

Display prerequisites and todo prerequisites fields.

Actual behavior

Prerequisites and todo prerequisites fields are missing
