bug: mcp-config.md and squad.agent.md refers to non-existent tool discovery skill

The mcp-config.md and squad.agent.md files refer to .squad/skills/mcp-tool-discovery/SKILL.md but I can't find any trace of the skill actually existing in the squad repo. Sorry this isn't an extensive report but just wanted to flag it and i'm not sure it's more appropriate to remove the reference or actually create the skill.

Thank you for catching this documentation inconsistency! You're right — the references to .squad/skills/mcp-tool-discovery/SKILL.md in mcp-config.md and squad.agent.md point to a skill that doesn't exist in the repository.

This is being addressed as part of the broader skills framework refactoring (see #798). The skill catalog and documentation are being realigned to match the current structure. Once that work ships, these references will either be removed or updated to point to the correct location.
