Example Module template

I added a wiki page with a draft of an example template
Suggestions/corrections can go in this issue

Over the next few weeks, please make sure your assigned modules conform to the template.
Some modules may not lend themselves well to the same template as the rest; use your best judgment to handle those on a case-by-case basis. The goal is to make the formatting predictable so that the only major difference between pages is the topic.

Need to decide on how we want the template to be formatted and update the template currently in How_to_Contribute.md to include adjustments from today's discussion (certification, etc)
