handle disconnects

The bot should handle being disconnected from the server by:

• reconnecting
• sticking an entry in the log for all active channels indicating it was disconnected, and reconnected

And maybe this mechanism should be the same one that handles the bot itself crashing, i.e., a mechanism that has the bot save state (including channel set, maybe also configuration).
