Rewrite publish/materialize and tracker semantics for always-tracked features and issues

Make features and issues explicitly GitHub-tracked before execution, keep tasks out of DEV_MAP runtime ownership, support safe sync-all-children only for already-materialized child issues, allow imported GitHub issues to move into a feature, and preserve stable IDs without migration history.
