Add GitHub templates for issues and pull requests

In my opinion, it would help this project if issues were described in a consistent format.
A good start would be to have templates for bug reports as well as feature requests.
Pull requests could also be standardized if there were a template for them too.

Steps involved:

• Create a bug report issue template
• Create a feature request issue template
• Implement pull request template file in .github folder

Feel free to open a PR with some suggested templates, then people can review and suggest changes.
