Add "How did you hear about the Foundation? Why are you joining?" to DNF Forms

Membership Form - Can we add a “How did you hear about the Foundation? Why are you joining?”

@codehippie1 Should it be a drop-down? especially for the "Did you hear about the foundation?" and for the second one, it can be just a text field where they write the reason why they are joining the foundation.

So, we have two fields. Or could you tell more about how you want it to look like?
