There are 2 concurring listing of Chapters - let's merge them

I have noted that there are 2 concurring listing of Chapters on the erpnext.org website.

1. https://erpnext.org/chapters
2. https://erpnext.org/chapter-list

They do not even list the same Chapters, so it's not just a duplicitz of URLs but apparently separate objects (example: German Chapter only listed on https://erpnext.org/chapter-list)

Let's merge those 2 lists and put the result under https://erpnext.org/chapters

I have updated the erpnext.org/chapter-list entry with new link to a meetup group as requested by @asbasawaraj as per this PR #375

However we still have the situation of 2 concurring list. I guess the erpnext.org/chapters is being generated from an ERPNext doctype, while the erpnext.org/chapter-list is hard coded.

So I assume we should deactivate the chapter doctype or so in order to remove the concurring list?
@asbasawaraj do you have the admin power to do that?
