Fix skillsaw CI failures

In the merge queue, skillsaw started failing CI (but merged anyways) because it wasn't a required check yet.

Fix the skillsaw errors and warnings so that it reports green in CI. We'll make it a required check manually after that merges.

Recommended approach

1. Run make lint to get the full list of current violations.
2. Fix source-level violations where possible:
   • content-weak-language in skills/code-implementation/SKILL.md (lines 161, 195): replace vague terms like "properly" and "note that" with specific, actionable language.
   • content-description-routing in skills/pr-risk-assessment/SKILL.md (line 3): add "Use when ..." trigger phrasing to the skill description.
3. For violations that are intentional (e.g., large skills exceeding context-budget limits), update the baseline with make lint-baseline.
4. Verify make lint exits cleanly before pushing.

Note: the baseline uses ceiling mode for context-budget violations — if a baselined skill's token count grows further, the violation will resurface. Keep skill sizes stable or raise the configured limits in .skillsaw.yaml if growth is expected.
