Here's an interesting case where I'm reviewing working changes - no branch. I've not modified these files, but I'm getting suggestions for them.

Maybe it’s reviewing everything it sees rather than just the changes?

Yes, I've mentioned this in our discussions before — since the prompts push it hard to find any issues, it tends to also notice them in the context code.
