chore: add a pull request template

Problem

Pull requests need a consistent review format for Wave contributions.

Scope

Implement this as one focused change in devx. Keep unrelated refactors out of the pull request.

Acceptance Criteria

• Add summary, related issue, test plan, and out-of-scope sections
• Add privacy and security impact checkboxes
• Require before/after evidence for visual changes
• Include a contributor checklist and Closes #N guidance

Test Plan

• Open a draft PR locally or from a fork and confirm the template renders correctly.
• Existing checks for the affected workspace pass.

Out of Scope

• Unrelated product features or broad dependency upgrades.
• Production deployment or changes to live credentials.

Before starting, apply through Drips Wave and wait to be assigned. The pull request must include Closes #, test evidence, and any relevant screenshots or security notes.
