Feat/move custom button from action to form body

• Dynamic Hover Tooltips: Updated the status tracker to display "Pending Since" or "Responded" timestamps on hover for each stage.
  • Header Lifecycle Dates: Added the record's Creation Date and Closing Date (if applicable) directly into the document header for quick
    reference.
  • Button Migration: Moved all workflow buttons (Raise Request, Submit Response, Close Query, Send to Stage, Reopen Query) from the
    "Actions" dropdown menu to the main header for immediate visibility.
  • "Send to All" Feature: Added a new "Send to All" button for Draft queries that triggers all stages simultaneously, shares document
    access, and sends individual email notifications using your existing templates.
• Duplicate Prevention: Added backend validation to prevent creating multiple Audit Level configurations for the same Branch and Division
  combination, while still allowing the same branch to have different divisions.
