[Web Bug]: input and textarea in playlist dialog panel not resetting

Description

When creating a new playlist immediately after having created a previous one, the popup panel requesting a name, description, and visibility will contain the name, description and visibility of the previously created playlist. This only happens when creating playlists back to back. Tested on Firefox. Minor interface bug. Not critical. The residual text can be overwritten and replaced.

Steps to reproduce

1. Create a playlist. Supply a name and description and visibility. input and textarea elements should not contain any text
2. Immediately after, create a second playlist. The input and textarea may contain existing text. These should be cleared or reset.
