Sounds similar to #971. I just posted test results there for 0.3.8.dev3 across a few model + cache configs. Could you take a look and let me know if your symptoms match? If they don't, please share the model name and the omlx version you're on so I can dig into yours separately.

v0.3.8.dev3 just shipped with several stability fixes that could explain the bad output you saw: a sampling determinism regression (every request was returning the same tokens at temp > 0), Qwen3.5 / Qwen3.6 forward alignment with mlx-lm, and sliding-window attention dilution.

If it still reproduces, the model name and a small sample of the bad output would help narrow it down.
