An unexpected error occurred.
For a spreadsheet file where the headers saying "Question", "Response", etc. are not at the immediate top. Sometimes there are titles or other instructions above the actual headers. It should be more resistant to this case and scan down the document until it finds a row that looks like a header row. The below screenshot is an example:
