I'd love to suggest this feature. Ideally, this is a similar functionality to how Apple's Books app (or other ebook apps) let the user create highlights and notes associated with sections of content - they can curate their notes and have easy access to refer back to their notes in context.
For the Moodle Book, I'm not sure about the database side of managing student annotations, but in terms of the UI it may be easy enough to detect the mouse over of a block level element and show a little icon to the right hand edge (make a note). Maybe notes could be managed or accessed in a similar way to the TOC. There's lots of possibilities. A more simple approach might be to allow one note per chapter.
