Oversized Tables and Figures - continued caption

Requirement (pg. 6-7 of style guide): "Subsequent pages have the table or figure number and the word 'Continued' and necessary column headings for ease of reading."

Shawn's approach: I had to split the tables/figures inside the LaTeX and used \ContinuedFloat and with an additional \caption containing the word "(Continued)" to do this.

I used the approach noted here with tabularx to define:

• a header for the first page by \endfirsthead
• a header for all next pages by \endhead
• a footer for all pages expect the last one by \endfoot
• a footer for the last page by \endlastfoot
