Record skill checks by assessee

Background

We currently have two methods for recording skill checks. Single independent checks and individually within a session.

Request

Develop a new page for recording competency by assessee (within a skill check session).

Requirements

1. An assessor selects a assessee (from those assigned to the session).
2. A table of the assigned skills is shown with form inputs to enter a competence level and optional notes.
3. A save button that saves the entered values.
4. A reset button that clears the entered values.
5. If an assessee is selected that already has checks recorded against it, populate the inputs with those.
6. Inputs are disabled if an assessee has not been selected.
7. The Save button is disabled if no values have been changed.
