[Bug]: LLM Provider model provider configuration cannot be updated

Description

Once configured, model provider cannot be updated.

Steps to Reproduce

1. Create LLM Provider using the Azure AI Foundry template
2. Notice how the selected provider is now set and cannot be updated (regardless of whether the changes were saved or not)
