Deploy organization-wide issue forms

What changed

• adds organization-wide bug, feature and generic implementation Issue forms
• adds a Product Epic form with an explicit product-planning-only authority boundary
• requires Features/implementation work to link one primary Product Epic
• carries acceptance, dependency, tenant/security and retained-evidence fields into executable work
• documents the hierarchy in the community-health README
• adds retained Issue-form YAML validation

Dependency evidence

The prior "Accepted operating model" blocker is satisfied by completed product-planning PLAN-01 and the current Product Operating Model.
