Permit reuse of a complete successful A38 report after policy approval when the exact tested head, base, recorded execution inputs and measured policy are unchanged. Revalidate the original report against the approved policy and perform a fresh live assessment; preserve its original timestamps, results and durations. Changed or unproven inputs still require a new complete run.

This change updates only the central process documentation. The existing guard already accepts matching evidence after approval.

Validation: GitHub CI passed on this exact head (1,128 passed, 1 existing skip); all nine relative documentation targets resolve. Independent final reviews were explicitly waived by the requester for this session; zero independent final-review approvals are claimed. Human merge is still required.
