Reopened #894: my earlier close was premature -- I checked docs/index.rst and
missed that the README has its own copy of the same paragraph, with its own
link.

README: the `setup`_ reference pointed at install.html, whose page is titled
"Installation", so the link text named a section that doesn't exist. Renamed the
target to `installation`_ so text and destination agree.

docs/index.rst: the cross-reference pointed at the "use" page. The original
broken link (an internal #setup anchor) and the README both meant installation,
so it now points there -- :doc:`installation <install>`. Verified by building
the docs: the emitted link is href="install.html", and the warning count is
unchanged from master at 7.

Also moved the three readthedocs URLs from the legacy .org domain to the
canonical .io.

Note for #894: GitHub's .rst viewer does not understand Sphinx roles, so a
:doc: reference will always render oddly when browsing docs/*.rst there --
that part is not fixable from here. The README, which GitHub renders as the
project homepage, now carries a plain absolute link that resolves correctly.
