Release v5.4.5 visual stress test

Summary

Release v5.4.5 is the visual stress-test pass requested after the professional filmmaker update.

• Adds seven generated PNG assets: two cinematic hero shots plus five text-rich infographics for skill capabilities, CDN delivery, reference roles, production delivery, and QC.
• Updates the README hero, visual gallery, badges, design standard, changelog, docs, eval metadata, and all active skill metadata to v5.4.5.
• Strengthens design validation so the repo now fails when the gallery is missing, PNGs are invalid or too small, or text-rich infographic guidance goes stale.
• Adds a dedicated eval case for the six-plus-image gallery requirement.

PR #5 was already merged, so this is a fresh PR from codex/v5-4-5-visual-stress-test containing only the v5.4.5 commit ahead of main.

Content audit still reports the migrated archive warning about an old likeness-policy phrase, but active guidance passes and the archived finding is warning-only.
