feat: approve reference voyage and port core facts

Summary

• Authentic project-owner approval applied for scoped statements STM-0395..STM-0410.
• 16 statements promoted to EvidenceCondition.SUPPORTED / HumanReviewState.APPROVED / PublishStatus.PUBLISH_ALLOWED.
• Canonical Shanghai (port:unlocode:CNSGH) and Tokyo (port:unlocode:JPTYO) port linkages retained strictly as Method.INFERRED with input closure (STM-0395 + STM-0405 and STM-0397 + STM-0408) and deterministic rule hash (211ffc44...).
• Minimal core fixes added to TruthEngine, Gatekeeper, and PortIntelligenceEvaluator to safely handle Method.INFERRED provenance without empty registry lookup.
• Private Source ART-0007 remains private_source = true; absent private bytes are NOT freshly cryptographically verified in CI (verification_status = PRIVATE_ARTIFACT_REFERENCE_REGISTERED), and global private-source publication safeguards remain intact.
• Fail-Closed Terminal Gating: Q-0038..Q-0041 (voyage.departure_terminal, voyage.arrival_terminal, voyage.departure_berth, voyage.arrival_berth) remain strictly UNKNOWN.
• No out-of-scope data: no voyage terminal, berth, route, geometry, or final cabin assignment introduced. Venue and cabin-feature statements remain untouched.
