Goal

Close the semantic-completeness gap exposed when PR #346 initially accepted a compound acceptance criterion with incomplete named proof.

What changed

  • Atomize every material criterion and trace each behavior to owner, implementation, named proof, custody, and result.
  • Require residual escape falsification before PASS.
  • Advance the advisory receipt to schema v2 and fail closed on incomplete proof.
  • Add specialty probes to all nine reviewer skills and custom agents.
  • Enforce stable semantic requirements independently for skills and agent TOMLs with mutation proof.
  • Add the realistic PR Plan bounded ContributionPolicy behavior #346 omission fixture.

Boundaries

No product behavior, migration, hosted receipt custody, merge authority, automatic merge, universal fanout, CI weakening, or coverage weakening. GitHub remains the sole merge authority.

Verification

  • All 10 updated skills validate.
  • 35 focused tests pass.
  • Reviewer validator, state synchronization, active projection, Markdown links, stale scans, and diff checks pass.
  • Adversarial probes reject authority fields, executable evidence, incomplete/unavailable trace rows, unverified second rows, and surviving/unavailable residual escapes.

Exact-head internal review

Head: a814da7715ea32a055413a2de6a5223a214c136e.

  • Architecture: PASS.
  • Security: PASS.
  • CI integrity: PASS.
  • Documentation: PASS.
  • QA/test delta: PASS WITH LOW RISKS.
  • Senior engineering/reuse: PASS.

All prior findings were replayed. Four valid CodeRabbit findings were fixed. The request to store in review in durable state was rejected because AGENTS.md requires this PR to land its declared final outcome and prohibits review-temporal states on main; GitHub PRs are the transient-work view.

Remaining low risk: structural enforcement cannot replace reviewer judgment about natural-language semantic completeness.

Human review focus

Confirm schema v2 and shared protocol fail closed without creating another authorization or merge system.

Review Change Stack

📝 Walkthrough

Walkthrough

The review-evidence integrity workflow now requires atomic behavior traceability and residual escape analysis. Receipt schema v2, reviewer guidance, agent contracts, initiative records, evaluations, and validation tests support these requirements.

Changes

Review evidence integrity

Layer / File(s) Summary
Initiative contract and evaluation
.agent-loop/CURRENT_STATE.md, .agent-loop/initiatives/WS-CI-004-review-evidence-integrity/...
The initiative records WS-CI-004-05 as complete. Its chunk contract defines semantic-completeness criteria, and QA evaluation data covers compound traceability.
Receipt schema v2
.agent-loop/templates/INTERNAL_REVIEW_RECEIPT.schema.json
Receipt schema version 2 requires traceability and residual_escape. Successful verdicts require verified traceability and a falsified residual escape.
Review skill completeness rules
.agents/skills/*
Review skills add behavior decomposition, ownership and proof mapping, domain-specific completeness checks, and residual-escape reporting.
Reviewer-agent enforcement
.codex/agents/*, scripts/reviewer_contracts.py
Reviewer instructions and contract checks require atomic evidence and block PASS for missing or narrative-only traceability.
Schema and contract validation
scripts/test_review_target.py, scripts/test_reviewer_contracts.py
Tests update fixtures to schema v2 and cover required fields, verified traceability, falsified residual escape, provisional verdicts, and positive findings.

Estimated code review effort: 4 (Complex) | ~45 minutes

Merge Risk: 🟡 Moderate · up to 0996b

The change strengthens review proof requirements, but the current head still has unresolved risks that could allow incomplete semantic evidence or premature completion state to be treated as authoritative. Merge should wait for explicit owner follow-up on enforcement and completion-state evidence.

In @.agent-loop/CURRENT_STATE.md:
- Line 47: Update the WS-CI-004 status row in the durable main-state table so
WS-CI-004-05 remains marked as in review until PR `#347` is merged; do not record
its completion there before merge, and preserve any completion claim only in
transient branch state if needed.

In
@.agent-loop/initiatives/WS-CI-004-review-evidence-integrity/chunks/WS-CI-004-05-semantic-review-completeness.md:
- Around line 48-65: Record completion evidence for every acceptance criterion
in the acceptance-criteria section: mark each verified item [x], or add a
completion table identifying its owner, implementation source, named proof,
execution custody, and result. Ensure no criterion is presented as complete
without corresponding verification evidence.
- Around line 67-75: Add python3 scripts/check_stale_workstream_wording.py to
the Verification commands list, and record “open sub-agent sessions: none” in
the applicable internal review evidence as a statement rather than a command.

Apply the same fix in @.agents/skills/docs-review/SKILL.md around lines 30 - 36:
The docs-review probe requests the same stale-wording and Markdown-link
verification evidence.

In `@scripts/reviewer_contracts.py`:
- Around line 89-92: Update the skill contract requirements and
contract_failures() checks to enforce every semantic-completeness clause:
atomization, ownership, implementation source, traceability, proof, execution
custody, result, residual escape hypothesis, and blocking PASS for missing or
narrative-only rows. Add mutation tests covering each individual required clause
so removing any one causes contract validation to fail, while preserving the
existing stable requirement identifiers.

Apply the same fix in @.agents/skills/reviewer-evidence-protocol/SKILL.md around
lines 25 - 40.

In `@scripts/test_review_target.py`:
- Around line 249-260: The tests must cover the same receipt boundary in both
scripts/test_review_target.py lines 249-260 and
scripts/test_reviewer_contracts.py lines 278-285: extend
test_final_verdict_requires_verified_trace_and_closed_escape (or its
corresponding test) so PASS rejects valid unavailable traceability and
residual_escape evidence, and add a second traceability row to verify every row
must be verified; retain the existing PROVISIONAL acceptance case for
unavailable evidence.
