feat: support OpenCode v2

Summary

• add native OpenCode v2 setup support while preserving the existing v1 server hook
• acquire the wake lock when v2 model work starts and release it when the session becomes idle, fails, or is interrupted
• use session.wait() as a completion fallback
• clean up session markers even when the platform inhibitor cannot start
• document both v1 and v2 configuration

Validation

• TypeScript build
• compiled module import smoke test confirmed both v1 and v2 exports
• package dry run confirmed the combined dist/index.js is published
