refactor: resolve clawpatch findings

Summary

• Consolidates duplicated WorktreeEntry and path-containment helpers.
• Removes duplicated CLI metadata/type state and redundant OutputWriter option mirrors.
• Deduplicates workflow configuration in commit-message and PR-review workflows.

Validation

• pnpm format
• pnpm typecheck
• pnpm lint
• pnpm test
• pre-commit hook checks on each commit: format:check, knip, typecheck, lint, fta, test
• ruby YAML parse for .github/workflows/commit-msg.yml and .github/workflows/pr-review.yml
• clawpatch revalidate for all 9 findings
