This is the first version of the Electronic Immunization Registry (EIR) DHIS-2 module.

The module is based on tracker and includes:

1. Electronic Immunization Registration Form
2. Appointments for future vaccinations
3. Adverse Event Reporting

Electronic Immunization Registration Form

This form is used to record information from each field encounter for vaccination of a registerd person.  During the encounter, the form collects

• Demographic information to record the person for the first time
• Information on the vaccine(s) provided, its dose, batch number and other related comments
• Basic information of the health status of the person.

Appointments for future vaccinations

This tab provides the opportunity to add future appoitments for the person.

Adverse Event Reporting

This tab collects information on adverse events possibly attributable to vaccination for the person
