fix: add create-pull-request to fix-review-findings workflow - #790

The agent couldn't push fixes because the workflow lacked create-pull-request. Also strengthened dispatch instructions with MANDATORY markers and direct tool names. Co-authored-by: Copilot <223556219+Copilot@users.noreply.github.com>

The fix-review-findings agent couldn't push its fixes — it was missing
create-pull-requestsafe output. Also strengthened dispatch instructions.
