fix(create): fall back to claim replay for claimed tokens

Summary

• treat inspect 409 TokenAlreadyClaimed as a fallback case instead of a hard failure
• let the CLI continue through normal claim replay and re-derive the target directory
• cover the claimed-token resume path in bootstrap preflight tests
