docs: align README with org template, add local dev tooling - #9

Move in design docs, a world-generator script, gui-maker tool, and resourcepack that were previously sitting in a stray sibling folder outside version control. Gitignore the local test-server directory (minecraft-test/) and generated world-gen output samples - neither belongs in source control.
