docs(readme): fix stale Claude Desktop/Code split FAQ from 5-skill era#7

The FAQ line "The Appeal Drafter and Denial Decoder skills also work in Claude Desktop ... The other three are Claude Code specific" was written when this repo had 5 skills (2 desktop + 3 CC-only). Recent commits expanded the surface to 8 skills, so "the other three" is mathematically stale. Rather than fabricate a new 2/6 split that has not been verified against actual Claude Desktop behavior, replace the line with the honest statement: the skills target Claude Code's slash-command install flow; the underlying MCP tools are still reachable from Claude Desktop directly when upstream-mcp is installed.
