Phase 3: show the mechanism
Adds a compact, fictional source → packet → response → review → decision → next-action example in English and Simplified Chinese.
The example deliberately shows an unsupported AI claim being corrected. It demonstrates selected continuity, human review and an inspectable project trail without representing a real LabNote deposit or promising automatic correctness.
Links are added from both READMEs and the documentation index. No schema, workflow or runtime behaviour changes.
