MCP-1F: Package the server and document setup, ops, and migration guidance

Summary

Package the MCP server so it can run independently of FastAPI, and document setup for real clients plus the operational caveats around remote media paths and embedding-dimension migrations.

Scope

• pyproject.toml
• console script such as agentic-memory-mcp
• dependency extras split for core, API, and MCP
• README setup for Claude Code, Cursor, Windsurf, and generic Streamable HTTP clients

• remote media-path caveats
• 768→1536 embedding-dimension migration guidance

Acceptance Criteria

• users can install only the dependencies they need
• users can run just the MCP server without starting FastAPI or Next.js
• README includes exact config examples for local stdio and streamable HTTP clients
• README documents that embedding-dimension changes are reindex migrations, not live in-place flips
