Add BranchToTitle helper that strips conventional prefixes (feat/, fix/, etc.), replaces hyphens/underscores with spaces, and capitalizes the first letter. Use it as the default PR title in both the TUI and CLI flows. Co-Authored-By: Claude Sonnet 4.6 <noreply@anthropic.com>
