[codex] Publish bridge flow updates

What changed

• Splits the app bootstrap/runtime into smaller modules while keeping the static script-loading model.
• Adds review playback, score overview updates, feedback controller wiring, and related UI/test coverage.
• Adds CI workflow coverage and Vijfkaart Hoog system documentation.
• Fixes submitted feedback so the form clears after a successful send.

Why

This publishes the current local bridge-app worktree so the broader flow, review, feedback, and architecture updates can be reviewed together.

Validation

• npm run test:unit passed: 379 tests.
• Targeted Playwright smoke passed earlier for feedback submit/copy flow: npx playwright test tests/browser/smoke.spec.js --project=desktop-chromium -g "can finish a hand and copy or submit a feedback report from the review".
