docs(changelog): consolidate 3.0.0 - #125

The [Unreleased] section held 21 separate '### Fixed' blocks accumulated over the review rounds, and a [3.0.0] section already existed below it — dated, but never tagged. Everything is now one 3.0.0 entry with a single heading per category, and this round's fixes are folded in: the X-Forwarded-For ACL bypass, the mTLS system-root fallback, the nil-process segfault, the orphaned-process regression, the autotune OOM, and the rest. The date moves to 2026-08-27 to match when the work actually finished.
