fix: discover .md workflow slash commands from .claude/workflows/#190

Adds scanWorkflowMdDir to sdk-workflow-discovery.js so that .md files in .claude/workflows/ are surfaced in the Clagentic autocomplete as user-invocable slash commands alongside the existing .js orchestration scripts. Extracts extractDescriptionFromContent from sdk-skill-discovery so the same YAML-frontmatter/prose description logic is shared without duplication. .md wins over .js on same-name collision; SKILL.md is skipped.
